You should inspect the walls of your fireplace to see how much soot is stuck on the walls. Scrape the side with your firepoker...if the lining or buildup is over 1/8th of an inch thick...you should schedule a cleaning.

If you use the fireplace more than once a month it should be cleaed annually.

Your chimney should be inspected annually, This will determine if it needs cleaned or not. In some cases they may not need cleaned every year, I have seen cases where they should be cleaned twice a year.
Have your chimney Inspected every year.
