Depends on your area, whether the existing subgrade is good or needs replacement too to provide proper support, whether you are talking a flat surface or specifically shaped panels for drainage or sloping, if there are embedded drain pipes through it, etc - but for your size job can typically run from under $3/SF in cheap labor and concrete areas to around $5-6/SF in higher priced or urban (tight access or long haul through city streets from materials plants) areas, depending on locale and the above factors. That is for normal concrete - can run up to as much as about $10-20/SF for unusual configuration (like warped shape or quite steep slopes) or for stained or stamped concrete simulating stone or brick.
