Assuming interior or roof sheathing (though rare to see 3/4 roof sheathing these days), materials about $30 plus or minus about $5 per 3/4" CDX sheet, plus probably about $5/sheet labor in quantity. For just one or a very few sheets, probably going to be charged the same amount above for the materials per sheet, and a minimum site visit charge of probably about $50-100 by a Handyman, or about $75-150 by a carpenter.


This of course assumes the joists or whatever you are going over has correct spacing for nailing, and does not require ledger or firring strips to correctly level and fasten it.


If exterior application (rare to see plywood used on exterior floor due to delamination issues) - 3/4" AB Marine plywood materials and exterior fasteners would likely be about $70-90/sheet for the materials - little or no difference in labor to do screws rather than nails. Similarly, probably about $50-70/sheet for interior A grade or veneered surface interior plywood if that is what you are looking for, though that would be very rarely done.
