Typically $90-200 service call charge depending on labor costs in your area - parts from $10-50 typically depending on whether replacing one component or all the replaceable parts. This assumes this is a normal 2-part tank type - if a powerflush or tankless type the parts may run quite a bit more - and I don't know if Toto is one of the type where a power-flush parts repair means dismounting the toilet and getting at it from the underside - if that is the case probably $50-100 more labor.

With a Toto - my sympathies - I strongly recommend asking him to use FluidMaster after-market parts, not OEM ones - the word is basically out on the Hunter and Guillerre (sp) as I recall OEM components in Toto's having a lot of reliability and functioning problems - both with the fill valve and the discharge flap.
