Likely you need not only shower head but the plumbing to it and the dioverter valve to get the water to it (though that might be stubbed out in the wall already), and likely a shower curtain or door too and also likely repair/replacement (and painting if backing wall) of damaged surround or backing wall which has to have holes put in it to install the plumbing to the shower head. - so I would say you probably need a Plumbing as well as a Ceramic Tile/Surround contractor minimum, or maybe a Remodeling - Kitchen and Bath general contractor to pull all those together in one contractor.

[He is going to have to, unless it happened to be pre-plumbed for a shower head, go into the wall at the faucet/valve to put in the diverter valve (if tub spout does not already have one that works) and run piping in the wall up to the shower head level, then the shower arm and the shower head - plus repair of those penetrations, and the shower curtain rod/curtain or shower door installation.]
