Got a couple of friends with muscle willing to help out for a pizza and beer afterwards ?

Otherwise:

1) some treadmills are easy to remove the ramp/tread from the upright portion, breaking it into two lighter parts

2) professionally, Moving (regular transfer and storage companies) is one Search the List category of vendor who can certainly do this - minimum charge commonly about $100-150 though.

3) Hauling - the "two guys and a truck" type junk pickup and disposal type company, also commonly do this - from $50-100 minimum charge commonly.
