It depends on what you require: a site visit or an online or phone consultation. For a site visit, I'd call the closest botanical garden and inquire. If online would work, try any cactus and succulent forum and make inquiries.

If you live in area where they are comon, your local plant center might be able to help with your problem.

If you think it is a disease or nutritiuon proboem, contact the Cooperative Extension Service - if not common in your state, try one in Arizona or New Mexico.

The National Park Service Saguaro National Park has online info on them and their care.

It is the state flower of Arizona, so the University of Arizona Cooperative Extension Service is guaranteed to have multiple brochures (almost always online) about planting and caring for them, and their common diseases and ailments and treatment of them.

You can google for botanists or arborists adertising their experitse with them by googling this phrase, and adding on your town name to the phrase - saguaro expert
