new construction concrete slab was wavy everywhere vct was installed over it floor looks bad who is responsible

We are building a church. New concrete slab was poured. VCT installer didn't notice right away that the floor wasn't level. It is pretty wavy everywhere. Concrete company examined the floor and said they would come in a grind the concrete level. They only did a couple of small areas and the VCT installer installed the tile. Didn't notice the floor looked bad till he turned off the lights. This floor looks bad, who is responsible to repair the concrete and flooring?
