Glass and Mirrors would be the normal Search the List category for this, with most types of windows - your normal Glass Replacement companies. For a few brands and of windows where they are built so replacing the glass is very difficult or impossible, a Window place that does repairs and new installations would be where to go.

Depending on type of glass and whether double or triple pane and whether it has special coating or such, I would guess cost range (assuming accessible and not over 2nd story) likely $150-300 range for single pane to $250-500 range for double or mylar-type triple pane, up to $500-1000 for high-end specially coated glass or true triple pane or safety glass. Possibly substantially less in low-labor-cost areas.
