How much to remove 12'× 36'×4" cement slab with 15×10" footer layed for a manufactured home?

Bid question - is this in open air or under a manufactured home ? I am assuming this is a slab-on-grade foundation.

Open air, I would say probably (including hauling away debris) around $1000 ballpark in cheap labor and disposal charge area to more in the $2000 range in higher priced area, to maybe as much as $3000 if tight working area with poor dumptruck access like in a backyard or tight city confines. Normal Demolition or Excavation contractor should be able to do this - be sure care is taken to shut off utilities first and have them properly cap off water and sewear lines.

If under the home - so jacking it up to provide clearance while the foundation is removed (and presumably then replaced - not included in this cost) probably more in the few thousand to more likely $5000 range - and a House Moving or maybe Foundation Repair company would be the normal vendors for this scenario.
