Many reasons - sounds like yours was overheating, either from pan going dry or pan not sitting flat on the burner (warped pan bottom or something on the burner or pan holding it up of the burner), or possibly burnt-on (carbonized) food on the burner - they are quite sensitive to that. A pan that leaks liquid (especially if cool or cold) or drips liquid like water onto a hot burner can do this too - like adding water to a hot pan at the sink and putting it back on a hot burner with some water on the outside of the pan, which drips down (or is on bottom) and makes contact with the hot glass.

Other possibility is the element shorted out and overheated the glass top as it failed - rapid temperature changes are what kills glass tops the easiest.
