Jiggle the tub/shower selector lever several times. Sometimes, especially as they get older, they tend to stick. Sometimes using a few drips of vinegar around the lever stem (vinegar and jiggling it around to free it up) can clear it up if sticking at the stem due to lime or soap scum buildup. If gummed up inside and putting drips of vinegar down the stem does not fre it up, basically you have to remove the lever (or tub spout if lever if mounted on that as is common) and acid clean it by submersion (which can be hard on the finish) or replace it.

Other common cause - if when you are done showering, you do not move the lever to tub spout position to let the water in the standpipe leading to the shower head drain out, the pressure from that 3-5 foot column of water can keep the valve which the lever operates closed - holding the water in the standpipe for hours and even sometimes for many days without it leaking out. Then, when you turn on the water faucet, the water comes out the shower head because the tub/shower diverter valve is already holding at shower setting - sometimes goes totally to shower, probably more commonly splits between that and the tub spout. Solution (other than moving lever to tub before or after water starts flowing) is to move the lever to tub position after each shower, to let the water drain out of the standpipe.

The reason this happens with cold but not hot water - I can only guess with hot water the fit between the tub spout or housing and the lever stem/valve is not tight enough (due to thermal expansion) to hold it in place, so it drops to tub position by itself - but with cold water it is able to hold in place. Jiggling the lever back to tub position should let it go to full tub spout flow - though might hve to turn water off to do that because otherwise in addition to getting sprayed in the process, you would be fighting the full water pressure on the valve.
