The difference is like going to Wal-Mart for a frozen dinner or going to a restaurant with quality, fresh food. You get what you pay for. Both may serve the purpose but one is going to be a lot better quality. I have never used Costco cabinets but they are likely modular construction as most are these days. Pay attention to whether the drawers are dovetail jointed or just butt joints. The dovetails are a lot stronger and will last longer. Also, make sure they are all solid wood. No veneers or particle board, especially in a bathroom. Often times there is not much of a price difference between semi-custom cabinets ordered from a store and full custom cabinets from a shop, depending on the options you choose. The difference is the quality. Custom cabinets are usually easy to touch up stain to closely match when scratched whereas factory finished cabinets are very difficult, if not impossible, to touch up. Weigh your options carefully and if the price is doable for custom cabinets I highly recommend them. Just make sure you pick a good shop that doesn't cut corners in construction.
