What is the most reliable, cost effective, and user friendly medical alert device to own?

In my experience, all the major providers of medical alert systems are highly reliable and generally are within the same price range of around $1 a day.

The things to look for are return policy, no long term contracts, US based monitoring center (not outsourced overseas) and no hidden fees.

From there you need to consider how you will use the system and if you need one that is mainly for home use, for people who wander, etc. Here is a good overview of the types of systems available on the market.

Your question is a good one because many meical alerts are cheap, but lack in reliability and quality, and others are amazing technology but they aren't affordable.

My friend recently got a med alert for his mother from ATS. He's very satisfied with it and he says it doesn't sacrifice any quality for its amazing price. It's very small, lightweight, and it can even be worn in the shower. It also has a very good range (over 500 feet), and the best part is, it doesn't have any monthly fees.
