Sounds like you need a seamstress - probably listed under alterations in Angie's List.

Since you say ones in th stores are all too small or big, unless you have unusually large neck or chest or long or short arms for your size, I would say go to a western shop and find one or more you like that fits you pretty good but may be a bit large in some areas, too small in NONE - then take it to a local alteration tailor to take in the places that are too big or long. That way you have hands-on measurements and markings on the actual shirt rather than having to get measurements somehow and send them to a remote worker, and you will know you have a shirt you like the and pattern and color of right off. You would be eliminating almost all the unknown, plus probably way cheaper than custom-made shirts.

One other alternative is getting professional measurements taken, and send them to a custom rodeo and western clothing outfit - google this search phrase - custom made western shirts - then check on Angie's List to see if they are rated or reviewed before doing business with them.
