Some Chimney Sweeps do minor repairs of this type - and could also expertly inspect for any problems with the chimney that might have caused the deterioration.

Otherwise, assuming just T1-11 siding or such, most Handymen will do this sort of work - as would probably a few Siding companies, though that small a job not many will be interested in.

If it is into the roof intersection, you might check around to see if you can find a Roofaer who will do it, because the flashing and such is more likely to be done right by a roofer.

LCD is on the right track in that several types of service providers can do this work.

Anecdotally, I had damaged wood at the top of my chimney replaced a few years ago by an exterior painting contractor. We had woodpecker damage on our chimney, but also a few other areas of wood rot, so it was part of a bigger job to repaint and repair wood on our home's exterior.
