Are you sure you need door replacement ? It might be possible to fix the leak with seal replacement while out unless the door unit frames are rotten too.

Repairs like this can be on the order of $1000-1500 for a door removal and simple sill area replacement, to several thousands if the flooring and subflooring are rotten, or if there is substantial wall/plate rot. Replacing the door unit will add $1000-1500 for normal economy units on up to as much as around $5000 for higher-end high-energy efficiency units.

You need a General Contractor with sliding glass door experience for this job. Put your emphasis on getting a reputable fair contractor, because until the tearout is well under way you will not know what the exact scope of work is, so you don't want a low bidder who is then going to nail you with overpriced change orders as the extent of the damage becomes evident.
