In our area, runs about $150 minimum, and for a normal 1500-2000SF or so roof about $250-350 in normal winter conditions, and $500-700 during or immediately after a major snowfall (a foot and a half to a few feet) when the city issues its snow danger alerts about too much snow weight on roofs, though I have heard of $1000+ bills in that instance when a person calls a crew out during the night or weekend because they hear popping trusses or rafters.

That is for snow and loose ice - removing ice dams is extra, as you do NOT want to chip away at them - causes significant damage to shingles even if you don't actually hit the shingles with the ice chipper. Best done with hot water AFTER you thaw the gutters.
